Athletic Participation Packet

At Forest City Christian Academy (FCCA), we are committed to providing a safe, organized, and Christ-centered athletic experience for all of our student-athletes. Before participating in any practices, tryouts, or games, each student-athlete must complete and submit the required participation forms.

Participation Packet

The FCCA Athletic Participation Packet includes: Athletic Consent and Release Form, Physical Examination Form, Emergency Contact Information, Athlete Code of Conduct Agreement. All documents must be submitted and approved by the Athletic Department before a student may participate.

After School Activity

Intramural Sports

Intramural Sports & Non-Participant Policy

No students will be allowed to be at intramurals after school unless they are signed up to play on that day/location. This includes siblings, carpool friends, and varsity sport team members.

Siblings, carpool friends, and varsity sports players who are not in that intramural sport may attend only if a parent or guardian brings them to the gym/field.

Rainy Day & Aftercare Policy

Aftercare Policy: On normal days when intramurals end at 4:20 PM, students who are not picked up by 4:30 PM will be taken to aftercare and the regular aftercare fee will apply.

Rainy Day Policy: On days when outside intramural activities have to be canceled due to weather, outside sports teams will wait with their classroom teachers at the dismissal area and then will be supervised in aftercare for "Intramural Aftercare" until 4:20. Intramural Aftercare is part of the intramural fee, so there is no added aftercare fee during the intramural scheduled time.
